The Texas Legislature passed House Bill 872, providing that certain customer information is excepted from disclosure under the Public Information Act. As a result of this legislation, BWA may not disclose personal and utility usage information for its customers unless the customer requests that BWA disclose such information on an appropriately marked form or other written request for disclosure.
If you want your personal and utility usage to remain confidential, you do not need to take any action.
However, If you prefer to have your personal and utility usage information available and released under the Public Information Act, please complete and submit this form to BWA.
Please upload a valid government-issued photo ID for the BWA Account Holder.
I request that Benbrook Water Authority disclose information on the above-referenced BWA water account when such information is requested under the Public Information Act. This request applies to information that (a) discloses whether services have been discontinued, or reveals whether an account is delinquent or eligible for disconnection by the government-operated utility; or (b) is collected as part of an advanced metering system for usage, services, and billing, including amounts billed or collected for utility usage. I understand that I must notify Benbrook Water Authority in writing if I wish to rescind this disclosure request.
By checking the "I agree" box above, you agree and acknowledge that 1) your Request for Disclosure of Information will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.
